Going on a first date? What should I do?

In all of my previous relationships i had completely skipped the dating portion. I just don't know what to do, say, bring, wear. I just need help on that if i were going on a date to either dinner, movies, hangin out at his house, or just studying (He hasn't exactly told me where yet). Plz hlp me... that and are girls supposed to ask or wait for the guy? If so how?
